Product description: TB6612FNG - 2-channel motor controller
Two channel H-bridge. Continuous current per channel 1,2 A momentary up to 3,2 A. Supply voltage up to 15 V. Housing SSOP-24 (SMD).

We also offer a module with TB6612 driver. The chip has holes for goldpin connectors (2.54 mm pitch), which allow to plug it into contact board or connect it by wires.
