There are no standards specifying which of these two elements should have voltage and where the ground should be, however, in recent years it has been assumed that the middle pin of a plug or socket is the so-called "hot" - there is the plus of the power supply, while the outer part of the connector is at ground level. Plugs are characterized mainly by two parameters - length and diameter. This ensures that connectors can be easily matched to each other without the need to perform more complex measurements.
